Consent for Endodontic Treatment / Retreatment
根管治療/ 根管再治療同意書

1. Irreversible pulpitis.
   無法修復的牙⿒發炎。
2. Necrotic pulp with/ without evidence of periradicular disease.
   牙髓壞死，可能伴隨牙根周圍發炎或問題。
3. Teeth with a pulp that would be compromised during dental procedures.
   在牙科治療過程中，牙⿒的牙髓受損。
4. Cracked or fractured teeth with/without pulpal involvement.
   牙⿒有裂縫或斷裂，可能會影響到牙髓。
5. Teeth with thermal hypersensitivity that significantly interferes with normal function, when alternative methods have failed to reduce the hypersensitivity.
   牙⿒對熱或冷的敏感度過⾼，且其他治療⽅法無法改善，影響正常咀嚼功能時，可能需要進⼀步治療。
6. Continued periradicular pathosis after root canal treatment.
   根管治療後，牙根周圍的問題仍然存在。

1. Hypersensitivity to medication and material used.
   對所使⽤的藥物或材料產⽣過敏反應。
2. Perforation or fracture of tooth while locating canals, removal of posts and separated instruments.
   在進⾏根管治療時，可能會因尋找根管、取出舊牙釘或清理斷裂的器具⽽導致牙⿒穿孔或斷裂。
3. Separated instrument during instrumentation.
   在治療過程中，⼯具斷裂並留在根管內。
4. Severely blocked, calcified or ledged canals where in canals preparations maybe left short if there is no evidence of periapical pathosis and further canal negotiation may be detrimental to the tooth.
   對於嚴重阻塞，鈣化或彎曲的根管，如果沒有根尖病變的證據，根管治療可能無法完全達到根管的末端，且繼續處理可能會對牙⿒造成損害。
5. Chance of tooth fracturing.
   牙⿒可能出現裂紋或斷裂的風險。
6. Damage to oral tissues or nerves.
   可能導致⼝腔組織或神經受損。

• Root canal therapy is a procedure designed to retain a tooth that may otherwise require extraction. Generally, root canal therapy has a success rate of about 80%-90%. Retreatment cases have a general success rate of 70% - 80%, however, results cannot be guaranteed.
  根管治療是為保存牙⿒以避免脫牙之程序, 此治療之成功率⼀般約為 80% - 90%, 根管再治癒的成功率為 70%⾄80%,但無法保證絕對成功。
• If complications that cannot be rectified or failure arises, surgical management or extraction are the available options.
  如果出現無法補救的併發症或治療失敗, 可以選擇⼿術治療或拔牙作為可⾏⽅案。

• Please understand that patient cooperation is very important factor contributing to the success of treatment. This cooperation entails refraining from biting on hard substances that could overload the teeth that have been treated, adopting good oral hygiene habits, and making regular appointments for dental check-ups.
  病⼈須明⽩治療的成功與否, 病⼈的合作是⼀項非常重要的因素, 這包括應避免使⽤根管治療後的牙⿒咀嚼堅硬食物, 保持良好的⼝腔衛⽣習慣，以及定期進⾏牙科檢查。
• Root canal treated tooth may be prone to breakage. If suitable, cusp coverage of the tooth can provide protection and prevent loss of tooth due to tooth fracture.
  經過根管治療的牙⿒比較較脆弱，容易斷裂。如果需要的話，裝上牙套可以加強保護，避免牙⿒因破裂⽽受損。
